Pdf document merge/maker

Hi

any suggestions for setup/ or product which will allow us to generate pdf documents from a form on a website running on unix server?

basically website form on a site.

is filled in and when sent off it can either

send an email in pdf format to client
or send information to database for retreival in pdf format
or send information to file and convert to pdf format for later retieval or automatically emails said document/s.

or other suggestion
