130+ MYSQL Process

One of my servers that had been running flawlessly for 40 days +, starting experiencing high loads, in excess of 50+, upon investigating I seen there were over 130+ mysql processes running, I rebooted the system (since at this point it was impossible to work with) and everything has returned to normal. The log show no indication of anything, anyone have any idea why this happened?

What would cause system to spawn 130 mysql+ processes?

Also, just to add, I had a similar problem on another system a few weeks ago where there were 150+ exim processes. Again totally random and unexpected.

root@cj [~]# mysql -V
mysql Ver 12.21 Distrib 4.0.15, for pc-linux (i686)
