Neomail on Cpanel: How to make a sign in form?

Hi there,

In order to check webmail on Cpanel, we go to:
http://www.domain.com/webmail/ or http://www.domain.com:2095

and a window will pop up to ask for username and password.

Now I want to make a sign in form in web page that lets user sign in directly to Neomail.

How to do this?
