separating email from hosting, how?

Let's say customer has a website + web-based email. Now customer wants to have email that is physically on their premises (MS Exchange, say, or sendmail on Linux).

What DNS changes are needed so that email to user@domain.com goes to the physical email server while HTTP requests for domain.com go to the separate website?

Whose DNS info is changed (the registrar's?).
