What is causing the packet losses?

I have a server that is running off ThePlanet network through LayeredTech and I am getting over 10% packet losses when running a continuous ping test.
Ive had several other users run these tests and they are getting similar results from different locations of the world.
The interesting thing is, some of these users who are experiencing high packet losses (including myself), when running a TRACERT, it doesnt show anything overly unusual. (some even showing consistent 68ms times) .....just getting packet losses every now and then.
So, my question is what could be causing this high packet loss issues on the server?
