How do I go about moving an entire website?

Sorry for the newbie stype question, but I've been ploughing through the search function and am struggling to narrow the results down.

I've been getting very poor service from my hosting company and have decided I have no other option other than to move. But once I've selected a new host, how do I go about actually moving the site across? Obviously I can take a total backup of the structure and forum database, but is there a way I can make the switch seamless or will there be an inevitable outage of some sort?
