what possible explanations for connection problems

Ok here's the scenario.

I have a FreeBSD server. I run ipfw to block all SSH execpt for IPs I allow.

I can ssh from ALL ISPs except for 1. On this 1, the server just doesn't respond. I don't know where the packet is being dropped. I can connect to the server using other methods, such as http. Also, it used to work. I made a rule to allow ALL incoming SSH connections, but this doesn't work. I have a feeling it's not the server. This has been going on for about two months.

Further problems. Today, I was setting up Outlook to use the smtp server on my box. Well, I can't connect from one of my ISPs. I try another ISP and this works.

What's the deal? Is my NOC randomly blocking ports & ip classes?? I'm 99% sure it's not the box. I'm looking at logs and nothing is being logged as far as the firewall rejecting packets.

Keep in mind http works fine from any ISP. What could be the cause and how do I troubleshoot more before contacting my dedicated server provider?
