how to swap hard drives

Hi,

I have a server with two hotswappable scsi hard drive bays. Well, I want to swap those harddrives into a new server with also has hot swappable bays, however, I've tried things like this before, and have always had to end up formating the primary drive, reinstalling centos3.3 and restore all the files from the backup drive. This is because it does a kernel error when finding the primary mount or something on /dev, or something similiar to that error. Basically, It can't find the startup harddrive or something, because it's not mounted in the same place or something.

When that happened, I looked through a bunch of forums, tried adding grub startup options that alot of people suggested, nothing worked. So now, if anyone could please tell me, what is the common procedure when planning to swap a harddrive from one linux server, to another, without complications?
