PHP mail() function stopped working all of a sudden - EMERGENCY

I haven't upgraded php in a while, have not touched /etc/php.ini . But php mail() across all hosted domains have stopped working across all my domains.

php-4.1.2-2.2
sendmail-8.11.6-28.72
mailscanner-4.28.6-1
apache-1.3.27-9.ent

Only mailscanner was upgraded in the past 24 hours.
