Mysql problems - Too many connections

What should be done in such a case when you have say 1 million connections and the site starts giving mysql errors

I have site which uses mysql as backend. Sometimes when the visitors become say 20,000 site starts giving errors.

Any idea what steps should be taken to minimise this?
It is already running on a dedicated server
