Swap repletion

I've just faced to the huge troubles with server...

TOP command shows that swap grow, grow and grow. At the same time httpd process hire too much cpu time.

After the lapse of some time server goes down, but not as usual. I can ping it but can't connect via SSH, http, ftp etc.

What happend?
Please, help me to find out the light in the end of tunnel.
