SSL works but no lock icon

SSL cert appears to be installed properly but no lock icon appears...?
No warnings ,lock icon just flashs while page is loading then disappears.

It is for www.mydomain.com
(lock icon shows for this using https://www.mydomain.com)

But when I try
https://www.mydomain.com/crt/final.php it disappears

why does the lock icon not appear?
Maybe images from a none secured file?
