Serving Images - Which Web Server?

Right now I am using Apache 1.3x and during peak load the web server load reaches 2 or 3. MySQL is on another machine. I would like to improve web server performance.

I have heard about thttpd, boa or using squid. Which is the best way to serve static images while still being able to disallow hotlinking? Thanks.
